Though the ghost NPCs on the radar might be stuff like crows and whatnot that fly away. I've also noticed green hash marks for NPCs that don't exist and disappear if I get close, and certain parts of the terrain (shrubs and other stuff you can't pick up) jump and then disappear. Going to VATS or Pipboy, or killing the enemy and going back to the game reduces this back down to one/none. I've noticed several times on my radar 3 or 4 enemies, but only one on the screen.

This mod requires New Vegas Script Extender and JIP LN NVSE Plugin to work. To install, download the file, extract it, copy the 'Data' folder to your Fallout: New Vegas directory, open FalloutNVLauncher, then click on 'Data Files' and enable the mod. One downside: NV actually did VATS quickly for me (remember that the mongrels at Bethesda introduced that 6 seconds VATS slowdown bug with one of their wonderful "patches?"), but now with the fixed dll, there's a two or three second slowdown again.
